About the Provider Network
What is the Provider Network?
The Provider Network is a collaborative group of providers across Polk, Hickory, and Dallas counties. It includes primary care providers, mental health professionals, social service agencies, civic and faith-based organizations, local government agencies, and others committed to delivering family-centered services.
All participating providers have signed an agreement to be part of the Network, which is facilitated by Barceda Families. The Network operates using a coordinated “lead agency” model to better support families.
What is the Lead Agency Model?
At Provider Network meetings, family cases are discussed (with proper consent through signed release forms) to understand their needs and goals under the CBCAP Project TEAMS initiative.
Based on each family’s situation:
- One agency becomes the lead agency
- The lead agency works directly with the family
- Other Network providers collaborate as needed
A team of providers may support the family, ensuring:
- Coordinated services
- Shared accountability
- Ongoing updates and documentation of progress
